1. Comprehensive Assessment
You complete intake documentation and mental health assessment materials. A Utah-licensed mental health professional conducts a thorough review to evaluate your current mental health status, symptoms, and functional needs.
2. Clinical Engagement & Care
You meet directly with your Utah-licensed clinician to establish an active, bona fide clinician-client relationship. Through this clinical care framework, your clinician diagnoses qualifying conditions, evaluates your therapeutic needs, and incorporates the emotional support animal into your formal treatment plan.
3. Documentation & Verification
If an emotional support animal is determined to be a necessary component of your care plan, official documentation is issued directly by your treating provider in compliance with federal and Utah fair housing standards. Letters remain valid for up to one year as part of your active care.
